The GLFHC and the YWCA have received $15,000 to support this program.

The Stoughton Youth Commission in collaboration with The Family Center,

Community Connections of Brockton is a recipient of the 2013 PLANET MassCONECT Institute for Community Health Program Planning (iCHPP) Grant Award.

The Stoughton Youth Commission and The Family Center, Community Connections of Brockton’s program entitled, “Promoting Healthy Eating in Lower Income Families,” will focus on building healthy eating practices by increasing access to fresh fruits and vegetables, educating on variations of food use, and promoting local farmers markets.  This project will attempt to cross barriers for families to use the farmers markets which will help low income families in the community and the local farmers.

The Stoughton Youth Commission and The Family Center, Community Connections of Brockton have received $15,000 to support this program.

The Billerica Public Schools is the recipient of the 2013 PLANET MassCONECT Institute for Community Health Program Planning (iCHPP) Grant Award.

The Billerica Public School’s program entitled, “TOPS,” will give students that have been recently discharged from the hospital, and those that are at risk for hospitalizations, the tools to succeed by letting them start their reintegration into academics out of the mainstream population. By doing this, the hope is to keep them in school and deal with their issues in order to get the education needed in our world.

The Billerica Public School received $10,000 to support program planning efforts.

All three proposals demonstrated a deep understanding of the iCHPP stepped approach to program planning (reviewing data, finding partners, exploring approaches, choosing and adapting a program and evaluating a program). Congratulations to Amy Weatherbee, Karen Hall, Carole Butze and their teams!
